New Patient Registration
New patients may register by simply filling in a registration form at the surgery and handing it over to us. Forms can be downloaded from the above "Registering Online" tab. We also require two forms of identification one of which should be picture ID.
Your application will be vetted by the doctor to ensure we can provide appropriate care for you. As a new patient you will be invited to attend the practice nurse to have a new patient medical carried out in order to identify your health needs in detail. If you do not have a medical card a completed registration form will suffice.
The practice services all areas of Hamilton (ML3 postcode).
Medical treatment is available from the date of registration. Please contact reception for further information.
Preference of Practitioner
Patients are registered with the practice, not an individual GP. For administrative reasons your medical card will be issued in the name of one of the doctors; however, you can at any time express a preference for a particular doctor for either all of your medical needs or on a case by case basis. We will do our best to respect your choice. However, not all the doctors in the practice provide all services and specific doctors may not be immediately available.
You may print off a registration form and the New Patient Information sheet, fill them out and bring it in with you on your first visit to the practice.
